How to Report a Claim

We understand that the loss of a loved one can be a very difficult and confusing time. That’s why we make it our priority to keep the claims process as simple and streamlined as possible. To report a claim, just complete the steps below.

1. Request a Claims Packet

You will need to complete a claims form and a W4P form in order for us to begin processing your claim.

By Phone

To request a claims packet by phone, call us at 1-800-303-6681. Our business hours are 8 a.m. to 6 p.m. (Eastern Time) Monday through Friday, or 9 a.m. to
5 p.m. Saturday.

Please have the following information available when you call: Contract Number or Social Security Number of Annuitant, Date of Death, and Cause of Death.

2. Complete and Mail Your Claim Form

Mail your completed claim form and a certified death certificate (copies are not accepted) to:

3. Provide Additional Documentation If Needed

If necessary, a claims representative may contact you to request additional documentation needed to complete your claim. For a complete list, see the Claims Checklist below. These forms, if requested, can be mailed to one of the addresses above.

Claims Checklist

Here is a list of documents you will need to submit in order for us to consider your claim:
1. Completed claim form
2. Certified death certificate (copies not accepted)
3. W-4P form

The following documents may be requested depending on your specific situation:
4. Proof of guardianship for a minor beneficiary
5. Executorship papers for the estate
6. Funeral home assignment(s)
7. Proof of death (i.e. newspaper article, police report, etc.)
